PRIVACY STATEMENT regarding CONFIDENTIALITY

Important points regarding confidentiality and my privacy commitment to you, and more specifically, the limits of my confidentiality.

Firstly:
All information disclosed to me will be held in strictest confidence; other individuals who may have access to your paper and/or electronic file, may include selected school professionals, but only on a ”need to know” basis. Please review the Canadian Counselling and Psychotherapy Association (CCPA) - Code of Ethics (available at the Guidance Office) for additional information on my privacy commitment to you.


Secondly:
Any information you disclose to me that may indicate that you could do harm to yourself or others will be outside the limits of my confidentiality. If there is imminent risk that you are seriously intending to harm yourself or someone else I would be required to take action to prevent that harm from occurring, including: alerting the authorities and/or warning the person who is being threatened; and/or contacting a third party.


Thirdly:
I am legally bound to report any information you reveal with regard to any suspected abuse or neglect of a child. As well, other abuse disclosures, such as abuse of the disabled and/or elderly.

Fourthly:
I am legally obligated to disclose any information that may be requested under a court order. For example, if I am called upon (subpoenaed) to testify about you in court my records about you could also be reviewed in court. Also considered here is when an individual files a complaint or claims professional liability by the counselor in a lawsuit.
